1. Coverage / basic building information

Collect BV information for Building-only or Package coverage that includes the Building. For BPP/Contents-only, it is not required. If uncertain, collect it anyway.

Coverage shown under Special Comments / “Need BV”

Building only Package — Building included BPP / Contents only Unsure / Need BV — collect details

Check the request for “Need BV” or coverage under Special Comments.

Building address

Building age (years)
Enter age or an estimate supported by records/contact.

Year built (if known)

Building area (total sq. ft.)
Use gross BUILDING area; not site acreage.

Number of stories

2. Exterior wall shell / cladding

Estimate the percentage of ALL exterior wall covering by material. Identify what each finish is installed over (substrate), where reasonably verified.

3. Occupancy, heating and air conditioning

Occupancy-use percentages and conditioned-area percentages are separate breakdowns. Each is based on TOTAL building floor area.

4. Interior finishes (finished sections)

For each different finished area, record FLOORING, WALL and CEILING finish; do not confuse the decorative ceiling with the roof structure.

5. Required photographs

Clear representative photos must substantiate the wall covering, flooring and ceiling materials described in the BV paragraph.

6. Narrative insert — separate BV paragraph

Click Generate BV Info to prepare the required stand-alone paragraph. Review and edit the wording before placing it in your inspection narrative.

BV Info: The building is approximately ___ years old and contains ___ sq. ft. of building area. The building’s shell wall covering is ___% ___ over ___ and ___% ___ over ___. The design has ___% air-conditioned ___ space and ___% non-AC ___ space. The flooring in the ___ section is ___, walls are ___ and the ceiling is ___.
See MGA example — separate BV paragraph

BV Info: The building’s shell wall covering is 75% stucco over concrete block and 25% Hardie Board on the front side. The design has 25% air conditioned office space and the remaining area is non AC warehouse space. The flooring in office section is vinyl tile, walls are sheetrock and the ceiling is acoustical tile.

Inspector notes / exceptions

Only use this area for useful clarifications, missing information or discrepancies.

Additional field notes

Inspector field guide — finishes and percentages

Exterior wall finish: specify both the covering and what is underneath when known. Example: “75% stucco over concrete block; 25% Hardie Board over wood framing.” If substrate cannot be confirmed, record that rather than guessing.

Interior floors: vinyl composition tile (VCT), carpet, laminate/LVP, ceramic tile, bare or polished concrete. Walls: drywall/sheetrock, CMU/block, wood paneling. Ceilings: acoustical/drop tile, drywall, exposed roof deck/structure.

Different percentages: cladding uses exterior WALL area; occupancy and conditioning use TOTAL building FLOOR area. Document estimates and their source.
