Field worksheet for documenting church operations, daycare and transportation exposures, life safety systems, premises conditions, and report-ready notes.
General Information
Building / System Updates
Daycare / Transportation
Underwriting Focus — Daycare and transportation
Daycare, field trips and vehicle transportation create additional supervision, licensing and premises liability exposures. Confirm enrollment, safety controls, vehicle use and who operates the transportation.
Building Design / Operations
Underwriting Focus — Commercial cooking / NFPA
Record hood/exhaust grease-cleaning and wet-chemical fire suppression service dates separately. Fire suppression maintenance is generally required at intervals of no more than six months (NFPA 17A and applicable manufacturer guidance). NFPA 96 grease-buildup inspection frequencies vary by cooking use: low-volume churches annually, moderate-volume semiannually, high-volume quarterly, and solid fuel monthly; cleaning is based on grease accumulation. Photograph each applicable service tag. Do not assume a current tag proves the system is clean or functional.
Church Features / Life Safety
Underwriting Focus — Candles and baptismal safety
For actual-flame candles, review noncombustible holders, distance from combustibles, supervision and extinguishing procedures. At baptismals, document handrails assisting entry and exit, step surfaces and applicable GFCI protection.
Underwriting Focus — Smoke detection and life safety
Identify smoke detector presence, locations and available test information; distinguish individual smoke detectors from the monitored fire alarm system. Verify extinguishers, sprinkler controls, exits and emergency lighting as applicable.
